Important: This screening form at the bottom must be filled out by each attending employee or vendors coming to the studio 24 hours or the same day prior to the meeting time. It cannot be filled out earlier than 24 hours. Each guest must fill out this form individually.
COVID-19 Protocols
- Maintain at least 1 metre or 6 feet distance from others.
- Masks or face coverings must be worn at all times.
- Cover a sneeze or cough with a tissue or bent elbow, & immediately dispose of tissue in a closed-lid bin.
- Stay at home if you are feeling unwell.
- Wash your hands frequently with soap & water, or with a hand sanitizer.
